چکیده

In hypertrophic cardiomyopathy (HCM) atrial fibrillation (AF) has been historically described as unrelenting, a driver for heart failure, with decisive impact on quality of life. AF treatment deemed less effective in HCM due to high rates recurrence. However, predominantly characterized from physicians perspective and there is no data patient reported outcome measures (PROMs). To evaluate the PROMs HCM. were collected using Atrial Fibrillation Severity Scale (AFSS) Kansas City Cardiomyopathy Questionnaire overall summary (KCCQ-OS) reflecting symptom burden life 250 consecutive patients. Among patients, 51 (20%) diagnosed at 56±10 years age, 6.5±5.5 prior evaluation health status. At evaluation, 45 patients (88%) sinus rhythm, 6 (12%) including 5 permanent AF. Each required symptoms: 27 (53%) solely medications (17 AV nodal 10 antiarrhythmic drugs); 24 (47%) ablation (16 catheter 8 surgical Cox-Maze IV time myectomy). Of 15 (63%) had recurrent (25%) requiring multiple procedures. After most recent ablation, was infrequent ≤1 episode per month 17 (71%), episode/year. 75% AFSS score consistent mild symptoms, 91% KCCQ-OS or disability. There difference either without recurrence after (p=0.68). The treated each episodes current therapy, but (56%) 11 With treatment, 67% symptoms 82% compared 199 (p=0.2). Recurrent common despite however tend be infrequent. majority have burden. These support that metrics are not significantly impaired

Management of Atrial Fibrillation in Hypertrophic Cardiomyopathy.

Case Presentation: A 38-year-old man with hypertrophic cardiomyopathy (HCM) was referred for management of intermittent palpitations and exertional dyspnea with routine activities despite escalated β-blocker therapy. His initial examination was notable for a regular pulse at 72 bpm and harsh systolic murmur that increased with the Valsalva maneuver. A 12-lead electrocardiography revealed sinus ...

The Impact of Atrial Fibrillation Ablation on Quality of Life.

Quality of life (QoL) is a very important endpoint in trials reporting the efficacy of catheter ablation in patients with atrial fibrillation (AF). It has been shown that AF ablation significantly improves the QoL, but recent studies question the usefulness of the most used generic QoL instrument in AF patients. The complexity of the disease makes it mandatory to employ disease specific instrum...

Hypertrophic Cardiomyopathy And Atrial Fibrillation: A Change

Hypertrophic cardiomyopathy was first recognized from pathologic studies in 1958 (1) and subsequently analyzed in detail from hemodynamic (2), echocardiographic (3) and electrophysiologic standpoints (4). It is characterized by a hypertrophied heart with a nondilated left ventricular chamber in the absence of any illness that induces an increased myocardial mass. It is frequently associated wit...

Impact of atrial fibrillation on the clinical course of hypertrophic cardiomyopathy.

BACKGROUND Clinical impact of atrial fibrillation (AF) in hypertrophic cardiomyopathy (HCM) is largely unresolved. Thus, we analyzed the prognostic implications of AF in a large, community-based HCM population assembled from Italian and US cohorts. METHODS AND RESULTS Occurrence of AF and outcome were assessed in 480 consecutive HCM patients (age at diagnosis, 45+/-20 years; 61% male) who wer...
